不良反應（英文：adverse effect），在醫學領域，又稱為不良作用、不良影響、不良後果，是指一種有害的，人們所不希望出現的，由於某種藥物或其他諸如化療或手術之類的醫療所造成的反應、效應、作用、效果、結果或影響。
